Voted : No, these things are inevitable
I dont think the pilots should be blamed but they seem to have precipitated themselves a bit. I've heard a expert saying they should have been able to recgnized the orange tags as belonging to friendlies. Pressure got the best of them methiks.

"friendly fire" incidents inevitably happen in wars. Usually, some one is to blame. Preventing it completely is pretty much impossible given that humans are flawed.

Yes, the two american pilots whose actions were at the very least grossly incompetent, and the american controllers on the ground. Its alright saying that these things happen in war, but American forces seem to make a habit of it, and all to often the victims of their mistakes are British or Canadian. Maybe the pilots would have taken more care if they had been in an area of American operation? The bahaviour of both the Uk and US govt has caused alot of anger- many Brits now realise the disdain we are viewed with by Washington. Special relationship my arse.

Advances in technology have reduced such instances to a record low in the US military, unfortunately they're still inevitable. Tragic, yes, but I wouldn't start handing out courtmarshalls just yet.
