I think the our gun laws, death penalty, gay rights and other issues cause people in other countries feel something is dramatically wrong with Americans, especially for example the gun laws and death penalty which MAJORITY of Americans support.
Seems as though US has been behind the other democracies on many human rights issues that go all the way back to slavery.
Not having available health insurance coverage for ALL Americans is in my opinion another human right we don't have and all of this might and maybe even justifiably, cause people in the other democracies to wonder WHAAAT we must be better than those damn yanks because look what we are doing and look what they are doing.
