doctors stitched you up after cutting you open.

you are healing well, and the threads are no longer needed to hold the wound together.

do you cut them off yourself, or do you go back to the doctor and have them pull them out?

I cut them out myself, I know how to operate a small scissors.
I let the doctors do it, scissors require a 8 year med. degree.

I used to let the doctor do it, but now if I ever needed removed, I would probably do myself or have one of my classmates do it for me.

Do it myself. I haven't had a cast since I was too little to choose, but I think now I would probably remove that myself as well. I'm not entirely sure how hard they are though.

I'd rather go ahead and let the doctor do it. He's the doctor for a reason and he could very well spot something that my untrained eye missed.

Though I have pulled out self-disolving stitches in my mouth before when they didn't completely come out. They just partially dissolved and were left hanging in my mouth, annoying the hell out of me.
