First three. Heart attack caused by drug overdose administered by an opportunistic doctor who killed the goose that laid the golden egg.

^Actually he didn't have a heart attack. He suffered cardiac arrest which is a completely different thing, and its what directly killed him, well, as far as we know. The drug overdose and medical negligence didn't directly kill him, though they may have and probably were contributing factors.
