I think a quick, deadly, highly communicable disease would be the worst.
Poverty is the worlds most prevelant weapon of mass destruction.

Extremely deadly disease (kills in hours), viral, aerosol-borne, extremely contagious, with a long dormancy period of say 5 years. Designed with a timer (number of generations of transference), so that the creators of it could be fairly certain of it's disappearance after awhile. Something like that could be spread around the world and kill billions when the timer ticked over. Saw the concept in a novel and it terrified me.
