I usually only hold doors for people that look like they are nice, so it's not usually a problem.

No, because where I'm at people still hold doors open out of sheer courtesy and habbit. It's just what you do out of kindness and you dont expect anything in return. Although the large majority do say thanks.

If someone's so worried about a thank you, they dont really need to be holding doors open to begin with.

I open the door for folks because it is part of who I am. I do it for me. If they express a bit of thanks, well so much the better.
