Usually, a few years after such places are moved into, the residents just fail to maintain the place as it should be. At least that's the way it was back in the day, when I lived in Da Hood.
maybe it's because people who are in desperate straits don't really care about their personal hygiene? also when people do get plastered, they tend to relieve themselves everywhere.

Because they are old buildings without proper ventilation usually in crowded neighborhoods. Lousy upkeep by the landlord certainly contributes.
Never been in a ghetto apartment, but the musty smell I think comes from the hallway carpeting. When you don't vacuum for a while the smoke and boiled potato and curry and piss all meld together into a wonderful cocktail of malodourous intensity. Maybe the landlords are too cheap to get a superintendant.
